REPORT CARDS AND ATTESTATION LETTERS

Report cards (for studies completed within the last three years)
If you are requesting a copy of report cards for studies completed at the Sir Wilfrid Laurier School Board within the last three years, please contact the school directly. Click here for our school directory.

Report cards issued more than three years ago and attestation letters
If you wish to obtain a copy of report cards issued more than three years ago or an attestation letter, you must complete the form below. Please note that an attestation letter indicates which school or schools you attended as a student of the Sir Wilfrid Laurier School Board as well as the specific school years.

For all requests, you must provide a proof of identity to be uploaded with the form below. Please note that once your identification has been verified, the copy of your ID card will be securely shred.

* Please note that preparing the documents requested may take up to 10 working days.

DIPLOMAS AND ACHIEVEMENT RECORDS
To obtain a copy of your secondary school diploma or an achievement record, you must submit your request to the Ministère de l’Éducation. Please click here to access the Ministère de l’Éducation website.

CERTIFICATE OF PREREQUISITES – VOCATIONAL TRAINING PROGRAMS
If you wish to apply for a professional training program in the construction field and need a certificate of prerequisites as required by the Commission de la construction du Québec (CCQ), we invite you to contact one of our adult education centres directly by clicking here.

ELIGIBILITY FOR INSTRUCTION IN ENGLISH
For more information regarding the procedure to follow and the documents required to be declared eligible for instruction in English in order to attend one of our schools, please contact our School Affairs and School Organization Department by clicking here.
